Our clinic is referral-based only. Only Dr Leonardi’s specific referral form (see below) will be accepted. Referrals sent without the referral form and required documents will be declined.

Patients can request a referral from their primary healthcare provider or gynecologist. Referral information should include previous imaging investigations (e.g. ultrasound and MRI reports), surgical records, and pathology records. Referrals without all the necessary information will be declined.

Referring physicians can fax referrals to 905-577-8377.

Monday (virtual) 8:00am – 4:00pm

Tuesdays (in person) 8:00am – 4:00pm
